We are continuing to work closely with NHS England and partners across the country such as the Greater Manchester Health and Social Care Partnership to scale up and realise the impact of personalisation.

This is tied closely to Health’s Lab vision of a people-powered health system that gives people and communities the power to be at the centre of their health, care and wellbeing.

We are currently providing advisory support to the Personalised Care Group in NHS England around the Integrated Accelerator Pilot announced by the Secretary of State in early 2018. This pilot aims to test an approach whereby every person coming forward for a needs assessment under the Care Act will be offered a proactive and joined-up approach to needs assessment and personalised care and support planning through multi-disciplinary teams.

Our other support around personalisation includes:

  • Supporting the adoption and rapid scale-up of Integrated Personal Commissioning across Stockton and Hertfordshire through 100 day challenges.
  • Working in partnership with local boroughs and the Greater Manchester Health and Social Care Partnership to scale person and community-centred approaches to care and support across Manchester (building on the chapter of the GM Health Population Plan).
