  • It sets out what we know about peer support, self-management education, health coaching, group activities and asset-based approaches, the value they can bring and a range of case studies and practical tips.

The most successful examples of person- and community-centred approaches in practice are those that are developed by people and communities, working with and alongside commissioners and policymakers to build on existing assets and co-produce solutions that work.

This catalogue of learning gives a practical insight into the work of five voluntary, community and social enterprise organisations, each with a wealth of experience of putting people and communities at the heart of health.

It was written for practitioners and local commissioners seeking to get to grips with the practicalities of implementing person- and community-centred approaches for health and wellbeing.
