This is an opportunity for local organisations or partnerships that are passionate about person-centred and community-based approaches in health and care to become a local partner site for Realising the Value.
We are looking for experienced local partner sites, from within the voluntary, community and social enterprise sector or that work closely with this sector, that have an appetite to use their existing experience to help others learn what works in practice.
We are seeking five local partner sites to each lead on one of the five areas of practice we have prioritised:
- Self-management education
- Peer support
- Health coaching
- Group activities that promote health and well-being
- Asset-based approaches in a health and well-being context

Becoming a local partner site would enable you to influence the national agenda on the role of person-centred and community-based care, receive high quality support to enhance your own work, learn from other experienced sites across the country and showcase your work on a national programme.
Local partner sites will work with our consortium to strengthen the evidence on person-centred and community-based health and care. This will include identifying barriers to implementation, helping to shape implementation tools, leading a community of interest and developing policy recommendations for the wider health and care system.
Local partner sites will need to be able to commit to working with us from November 2015 to June 2016. We offer a grant to cover the direct costs of involvement. In return, we ask for your commitment to shaping high quality tools and resources for others to use.
